5.06 I Believe the Children Are Our Future
A little girl places her tooth under her father's pillow instead of her own. The Tooth Fairy — as imagined by Jesse Turner — appears in the night to remove each and every one of the father's teeth. He also left 32 quarters under the man's pillow — one for each tooth.
8.06 Southern Comfort
Garth tells Sam and Dean he used to be a dentist, and it's where he got his first case as a hunter. Dean mockingly asks if it was the Tooth Fairy; Garth confirms that it was, telling the Winchesters that he felt terrible about it.
15.10 The Heroes' Journey
Garth reminds Dean that he used to be a dentist when he is about to examine his teeth. Dean recalls that Garth killed the Tooth Fairy, and he replies that "she had it coming."
Tooth Fairy in Lore
