History
Mary Worthinhton was 19, and lived by herself in Fort Wayne, Indiana. She had won a few local beauty contests and dreamed of getting out of Indiana, and becoming an actress. She was murdered on March 29th (year unspecified) when someone broke into her apartment and murdered her, cut out her eyes with a knife. Before she died, in front of a mirror, she tried to spell out her killer's name but only managed the letters T-R-E. A surgeon called Trevor Sampson was suspected but nothing was ever proved and her murder went unsolved. In revenge, Mary haunts the old mirror, killing people who hide secrets about unsolved deaths by burning out their eyeballs, unconcerned about the specific role her victims played in their own victims' deaths.

1.05 Bloody Mary
Mary targets both Jill, a woman who killed a young boy in a hit-and-run and covered it up, and Charlie, whose boyfriend committed suicide when she tried to leave him. After Sam lured her to him using his own guilt over not mentioning his dreams about Jessica's death to her, she was forced out of her mirror and destroyed by Dean when he showed her her own reflection in a mirror, causing her own power to kill people with secrets to destroy her. Dean then smashed the mirror.
14.20 Moriah
After Chuck decides to end things and releases spirits from Hell, Mary Worthington is one of the many freed, appearing in the mirror of two young girls playing dress up.
15.01 Back and to the Future
Mary Worthington attacks a young girl named Rachel, as she and her friend are trying on make-up, causing Rachel to bleed from her eyes and begin clawing at her face. When Rachel's friend tries to run for help, she is met by Mary standing in front of her bedroom door. When Castiel enters the home to evacuate the residents, he finds Rachel and her friend dead in the room, in a pool of blood. As he leaves the home, Mary appears in a mirror watching him exit.
As Sam and Castiel are leading a mother and daughter to safety, the daughter becomes distracted by a pond, and as John Wayne Gacy and two other ghosts start attacking Sam and Castiel, Bloody Mary appears in the pond's reflection and jumps out to attack the mother and daughter, grabbing the mother's leg. As they break free of Mary's grasp, Mary pulls herself out of the pond, only for Sam to shoot her with rock salt. She soon joins with the other spirits and gives chase to the four, but is stopped when Sam and Castiel get the mother and daughter across the barrier Belphegor created to keep the ghosts inside Harlan, Kansas.
15.03 The Rupture
Rowena cast a spell on herself that absorbed all the Hell Ghosts into her body as she falls into the closing crevice leading to Hell, sacrificing herself to imprison every ghost in Belphegor's barrier, including Mary.
