Lillian was the daughter of Jim McAnn, one of the three owners of the Midwestern Brewing Company. She had a daughter called Tess.

After her brothers, Trevor and Ray, are killed under suspicious circumstances, Lillian is having a tough time. One night, she pours herself and her daughter a glass of orange juice and adds vodka to her own. She carries the glasses to the dining room and sets them on the table where Tess is drawing.

Distracted by her father entering the house, Lillian walks off to greet him. Tess picks up a glass and takes a sip before realising it was her mother's instead of her own. Lillian goes to the kitchen to get some snacks for them. Soon after, Tess walks to the kitchen and points in horror. Lillian turns around and gasps as she sees a pale woman with sunken eyes and long black hair in a white dress. She turns around as if to run, but the ghostly figure shoves its hand through Lillian's back, killing her.

It is later revealed that the woman in the white dress was a shojo which Dale Lampert had sent to kill the children of Jim McAnn and Randy Baxter after they sold Thighslapper ale, a drink he'd created, to a large distributor against his wishes, leading to his suicide. The shojo is eventually killed by Dean.
