Synopsis

Following the memories that resurfaced in 6.13 Unforgiven, Sam has collapsed. Dean frantically tries to rouse him, while Sam suffers through flashbacks to Hell. When Sam recovers, Dean is adamant that Sam take seriously the harm that remembering anything from the last 18 months could do to him. Sam reluctantly agrees. By way of a distraction, Dean has found a case - Dave a janitor has been found dead in a locked lab at the Great Falls Junior College, in Paterson, New Jersey

The Winchesters have no leads, when a similar incident occurs when a nightwatchman, Steve, dies at the Salzman & Sons Apparel factory in nearby Passaic. On investigation Sam notices the EMF meter detects readings near a mannequin, as it also did near an anatomy dummy at the lab. Sam suggest a ghost may have possessed the figures, although the boys are confused as ghostly are usually tied to a particular area. Further research reveals that a year ago, an employee called Rose Brown went missing.

Dean receives a call from Ben, asking for his help because Lisa is depressed and locked in her room. Sam convinces Dean to go, while he heads off to interview Isabelle Brown, the missing woman's sister.

At the Braden house when Lisa opens the door dressed for a date, Dean realises Ben has tricked him. He and Lisa talk, struggling to deal with the fact that despite their feelings for each other, it seems impossible for their relationship to work. Talking to Ben, Dean explains that he thinks he is a bad influence and that being in their lives will inevitably lead Ben to a life of hunting. Ben is upset, and charges Dean with abandoning his family. To this Dean has no reply.

In Passaic, Sam speaks to Isabelle Brown who describes her sister as quiet and shy, the sort of girl who was an easy target for teasing and pranks. Leafing through a photo album, Sam realises that the janitor from Paterson also used to work at Salzman & Sons. Back at the factory, Sam interviews workers, and one in particular - Johnny - seems very nervous. That night he is also attacked, but Sam intervenes. He confesses to Sam that he and some workmates did bully Rose, and lead her to believe she had a secret admirer. When she arrived at an apartment for a supposed date, and they revealed the prank, as she went to leave Dave grabbed her and she fell, hitting her head on a coffee table and dying. Sam finds out where they buried the body, and heads off to salt and burn the corpse. He calls Johnny, telling him the haunting is over.

That night Johnny returns to his apartment, ready to leave town as he explains to his Real Doll. She becomes animated and kills him.

Sam speaks to Isabel and she reveals that she had one of Rose's kidneys transplanted into her when she was 16 - that is how the spirit has moved around, able to go wherever Isabel does. Rendezvousing with Dean they are stuck as how to deal with the situation, wondering whether Hoodoo may provide a temporary solution. As they discuss option, the Impala revs up - it has been possessed, and starts pursuing Dean. He mangers to maneuver it into a wall, but in the crash Isabel is fatally injured. As she dies, the spirit of Rose appears and apologizes, before disappearing.

Back at Bobby's place, Dean works on the Impala. he questions the value of what they do, but Sam points out that, while things don't always work out, they do save lives, and they did stop Lucifer. He thanks Dean for getting his soul back, and reminds Dean that he will always have his back.

Dean: What are you suggesting - we cut it out of her?

Sam: And then what, leave her in a tub of ice with a phone strapped to her hand?
Sam is referring to a famous urban legend about the harvesting of kidneys Source. In the legend, someone has their drink spiked, and when they wake up they are in a tub of ice, with a phone strapped to their hand, and a note to call 911. When the do the operator asks them if there is a tube coming out of their back. When the victim confirms that there is, the operator sends paramedics, who find that the victims kidneys have been removed.
