Synopsis

Sam and Dean travel to San Francisco to investigate violent deaths that they suspect are due to a werewolf. They visit Madison, a lawyer's assistant (possibly secretary, or a paralegal), whose boss was the latest victim. She admits that he would hit on her occasionally, and that the night of his death she spotted her violent ex-boyfriend Kurt watching them at a bar.

The boys find signs of werewolf activity near Kurt’s apartment and discover the body of a dead police officer in a nearby alley. Fearing for Madison’s safety, they decide she needs protection -- a job for which each brother is keen to volunteer. A game of Rock, Paper, Scissors results in Sam being left to guard Madison while Dean is out investigating Kurt. There is an obvious attraction between Sam and Madison, and while Madison flirts, Sam is shy and they spend the evening watching TV until Madison goes to bed. During the night, unnoticed by Sam, Madison transforms into a werewolf and sneaks out to kill Kurt. Dean bursts in on her and manages to nick her with a silver blade before she knocks him out.

Sam interrogates Madison and figures out that Madison was infected when she was bitten during what she thought was a mugging a month previously. Sam finds lore in John's Journal that suggests that if the "sire" of someone who is a werewolf is killed, they will be released from the curse. While Sam guards Madison, Dean visits the area of town where the previous victims were attacked. He saves a young woman from an attack by a werewolf, which he kills. As it dies it reverts to human form and is revealed as Madison’s neighbor Glen.

Sam and Dean stay with Madison through the next night; she doesn’t transform, and they assume she has been cured. Buoyant at this outcome, Sam and Madison consummate their attraction with a session of passionate lovemaking. But that night, as the full moon rises, Sam witnesses Madison become a werewolf again before she flees the apartment.

The next day, Madison calls Sam, scared and confused. When Dean confirms that they have no way to cure her, Madison begs Sam to kill her, because she doesn't want to live as a monster. While distraught and reluctant, when there appears no alternative, Sam, despite Dean's offer to do it instead, kills Madison.

Madison: It’s not an act! I am not a werewolf! There’s no such thing! It’s made up, all right? Th– They’re not real! You know they’re not real!

Dean: Sam, she’s a monster, and you’re feeling sorry for her?
Sam: Maybe I understand her... Look, there might be another way we can get the job done without having to waste her.
Dean: Are you thinking what I think you’re thinking?
Sam: Dad’s theory – 'lycanthropy might have a cure if you kill the werewolf who bit you, severing the bloodline.'
Dean: Might have a cure. Meaning who the hell knows?

Sam: It’s worth a shot.
Sam: Look … I know you’re scared. I also know that there’s no way in hell you’re gonna believe me. But I’m doing this because I’m trying to help you. I’m not gonna lie, all right, the odds aren’t exactly in our favor. But if this goes the way I pray it does... I’ll untie you, and I’ll walk out that door, and I’ll never come back. You’ll live the rest of your life. And I’ll just be a bad memory.
Dean: It was sort of sad, actually. Glen had no clue what was going on. Hey, why do you think he turned Madison instead of just killing her in the first place?

Sam: I don’t know. I mean, he kind of seemed to have a thing for her.

Dean: Maybe his primal instinct did, too. Maybe he was looking for a little, uh, hot breeding action.
Madison: Oh, God, thank you. Thank you so much. You, too, Dean. Thank you.

Dean: Aw, don’t mention it... So, I’m just gonna head back to the hotel and... watch some Pay-Per-View, or something.
Madison: That was smooth, heh.
Sam: He means well, but...

Madison: You mean, he thinks you’re gonna get laid.
Madison: Sam… I’m a monster.

Sam: You don’t have to be. We could find a way, all right? I can. I’m gonna save you.
Madison: You tried. I know you tried. But this is all there is left. Help me, Sam. I want you to do it. I want it to be you.
Sam: I can’t.

Madison: I don’t wanna die. I don’t. But I can’t live like this. This is the way you can save me. Please. I’m asking you to save me.

Trivia & References

Dean introduces himself and Sam as "Detectives Landis and Dante." John Landis directed An American Werewolf in London, and Joe Dante directed The Howling - both 1981 werewolf horror films.
Kurt Mueller is initially suspected to be the werewolf. The Turkish word kurt means wolf.
Dean: Think he's our Dog-Faced Boy?
Fedor Jeftichew - better known as Jo-Jo the Dog-Faced Boy - was a famous Russian sideshow performer brought to the U.S. by P.T. Barnum in 1884.
Dean: I'd say Kurt's looking more and more like our Cujo.
Cujo is a Stephen King book (and later movie) about a rabid St. Bernard named Cujo that kills people.
Sam: Wait, so, so, Kendall married Ethan’s father, just to get back at him?

When Dean pumps his fist in the air when he leaves to give Sam and Madison some alone time, this could be reminiscent of the iconic last scene of the 1985 movie The Breakfast Club where the character John Bender does the same gesture.

Sam: Dean, always with the scissors.
Sam and Dean's game of Rock, Paper, Scissors is referenced multiple times throughout the series. Dean always "throws" scissors, which Sam knows, so Sam always "throws" rock to win the game. See 6.17 My Heart Will Go On for an alternate universe reversal of this constant of the Winchesters' lives.
Madison is the second love interest of Sam's to die, the first being his girlfriend, Jessica Moore, who is killed by Brady in 1.01 Pilot. See also Peen of Death.
