Outline | Dean continues to become more aggressive because of the Mark of Cain, forcing Crowley to seek out unlikely help. After escaping Cole, Sam continues his search for Dean. Castiel's ever weakening state leads to Hannah seeking help from Metatron in restoring his grace. |

Synopsis
On June 21, 2003, Dean kills a man in a house in Nyack, New York. The man's son finds his father's body and sees that Dean is the killer. In present time, the son, Cole, explains this to Sam who he captured to use as bait to get to Dean. Later, Cole purposely lets Sam escape to get to Dean. Sam tries to explain that Dean isn't Dean and that there are monsters, but Cole continues to taunt and torture Sam.
In Killdeer, North Dakota, Dean is in a strip club where he harasses the dancer and beats up the security guard. At the same time, Cole continues to beat on Sam, demanding to know where Dean is. Cole is interrupted by a phone call, and Sam escapes. Meanwhile, Dean is confronted by Crowley who wants to talk about Dean's behavior.
After the fight with Adina, Castiel's grace continues to fade, and Hannah wants to help. Sam calls Cas and tells him that Dean is a demon, and he needs Cas' help. Sam tells him to get to Beulah, North Dakota as quickly as possible to get on Dean's trail while it's still hot. Hannah thinks the Winchesters are a bad influence on Cas, but they head towards Beulah regardless. Along the way, Cas falls asleep while driving and they have an accident.
Dean and Crowley sit at a bar and discuss the effects of the Mark of Cain. Crowley says the Mark has changed Dean and that Dean needs to feed the Mark by killing. He gets Dean to agree to kill a human as part of a crossroads deal. Crowley explains that Mindy Morris is a cheating wife who, upon discovery, wants a divorce and half of the assets. The husband, Lester Morris, made a deal to have her killed.
Dean goes to Mindy's house, but he spots Lester. He decides to have a chat with Lester, who it turns out is a douchebag and a loser. After Lester orders Dean to kill his wife, Dean instead decides to kill Lester.
Sam continues to track Dean, as Cole follows him. Meanwhile, Cas and Hannah are rescued by a tow truck driver and they spend some time at her house and with her daughter. Cas and the daughter have an interesting conversation about snot. The mom comments that Cas is a great catch, as Cas and Hannah continue on their journey.
As Crowley is being informed about Sam's whereabouts, Dean arrives to report that he killed Lester. Crowley is not happy that Dean killed the client. Dean makes it clear that he will do whatever he wants and that Crowley does not control him. Crowley backs down, but he secretly plots to turn Dean over to Sam. Crowley says that Dean is Sam's problem, now and forever.
As Cas sleeps in the car, Hannah returns to Heaven to talk with Metatron. She wants to know about Castiel's grace. Metatron taunts and teases, and he wants to trade Cas' grace for his freedom. As Hannah weighs her options, Cas arrives and refuses the deal. He says that Metatron is a liar and that deals made of desperation always end in blood and tears. Cas says it is his choice, and he doesn't want this. Metatron teases Cas that there is some of his grace left, but Cas has made peace with his fate and that he is happy that Metatron is locked up. Metatron threatens that he will get out and everybody will die.
At a bar, Dean plays the piano and replays Crowley's words, when Sam walks in. Dean wants to know who winged Sam in relation to his shoulder and Dean tells Sam that he knows what Sam has done in his attempts to find him. Dean wants Sam to let him go, but Sam can't do that. Sam points out that they know how to cure demons, but Dean says if he wanted to be cured he wouldn't have left. Sam blames Crowley and wants to fix this problem. Dean is giving Sam a chance to walk away, but Sam is going to pass. Dean says he is not walking out the door with Sam, so he asks if Sam is going to kill him. Sam says he will not kill Dean because Dean is his brother and he wants to take Dean home. Dean taunts Sam for being so emotional and having puppy dog eyes. As Sam approaches Dean with the binding handcuffs, Cole tosses tear gas into the bar. When Sam stumbles out, Cole knocks him out and confronts Dean.
Dean realizes that this is the guy that was going to kill Sam. Cole admits that he decided to let Sam escape so he could follow him to Dean. Cole explains who he is and that he has planned this for half his life. He intends to get revenge. The two fight. Dean toys with Cole and easily defeats him. Cole demands that Dean kill him, but Dean refuses. As Demon Dean is distracted by Cole, Sam burns Dean with holy water and slaps the binding cuffs on him.
Cole, who has now been introduced to demons, heads to the library to do some research.
Meanwhile, Sam keeps his end of the deal with Crowley by giving him the First Blade, as Dean watches from the back seat of the Impala. As Sam and Dean head to the bunker, Dean shows his colors by saying the Impala is just a car. However, Sam insists that Dean is still in there and that he showed mercy. Dean quashes that thought by telling Sam that Dean didn't show mercy — he did the exact opposite by letting Cole live. Dean then insists that he will not show mercy to Sammy either.

Castiel: Well, I, uh, I don't really dream.
Little Girl: Why? One time I dreamed that my snot was a rocket and it shot into space and knocked down the stars to make room for more rockets!
Metatron: You know perfectly well what she is doing, Asstiel.
Metatron: Yeah? Um, no I'll get out. It may take a century, it may take a millennium or two, but this dump will not hold me forever. And when I'm free, here's a little sneak preview: Everybody dies.
Sam: No.
Dean: Why? You don't know what I've done, I might have it coming.
Sam: I don't care. Because you are my brother and I'm here to take you home.
Cole: I saw you, that night after. You let me live. That was dumb, real dumb. I've spent half my life training for this moment, I've played out this fight a thousand times in my mind. And I know about you Dean-O, and you're good, you're real good. But you see, I'm better.

Crowley: Oh stop it, Samantha, nobody likes a tease.
Dean: It's just a car, Sam.
Dean: You call that mercy? Imagine you spend your whole life hunting down the guy that knifed your father. When you finally find him, he whips you like a dog -- how do you think that feels? That kid is going to spend his whole life knowing that he had his shot and that he couldn't beat me, that ain't mercy. That's the worst thing I could have done to him. And what I'm gonna do to you, Sammy, well that ain't gonna be mercy either.
