Synopsis

In a cabin in Lost Creek, Colorado, Tommy Collins—a victim of a wendigo almost nine years before in 1.02 Wendigo—looks out the window, despite his girlfriend Shelley's protests. He hears familiar growling noises outside and tells her it is a wendigo. Tommy grabs an acetylene torch out of his duffel bag, but he is flung against the wall by an unseen force. He begins to bleed from his eyes until his head explodes, spraying Shelly with blood.

Sam and Dean are researching the Men of Letters files in the bunker on demonic possession to try to understand how to complete the third trial. Sam admits to feeling unwell, but says that only completing the trials will help.

Dean brings Sam the food that is left in the kitchen and plans to go on a supply run. He refuses Cas’ offer to help. Cas apologizes for everything, but Dean tells him that an apology isn't enough since he lost the angel tablet.

Dean and Sam go looking for a Room 7B in the bunker. They look for files on Case 1138, a “Class 5 Infernal” event in St. Louis in 1957. The only note on the file was the word “weird” with three exclamation marks. Sam finds the file as Dean pulls away a shelf revealing a concealed room. There are chains etched with spell-work on the walls and a huge devil's trap on the floor. They open the file Sam found and find a film reel.

The brothers and Castiel set up an old projector to watch the film. A woman named Josie Sands is filming a young priest, Father Simon, who is performing a new type of exorcism. The brothers realize that Josie is actually the demon Abaddon's meatsuit. Father Simon and an older priest, Father Max Thompson, perform the exorcism using the same manacles that the brothers found in the hidden room. The older priest cuts his hand, forces the demon to drink his blood, and this expels the demon. However, the old woman the demon was possessing was dead. Thompson tells Josie to stop filming and the movie ends.

Sam and Dean travel to St. Louis to speak to Father Simon, who explains that the older priest believed that a demon could be saved by restoring its humanity. He explains that the demon escaped into the world, and Simon refused to take part in any more of Thompson’s experiments. Shortly afterward, something tore Thompson apart. When Castiel goes into town to shop, Metatron appears and tells him that they need to talk.

Back at the bunker, Dean and Sam watch the reel of the last test, in which Father Thompson works with a possessed man named Peter Kent. Father Thompson injects his own blood – purified by confession - into Kent’s neck with a syringe. He repeats this several times, each time asking the demon how it felt to eat Kent’s children. The demon finally begs him to stop and Father Thompson throws holy water on him, burning him, then cuts his hand and forces the blood into Kent’s mouth. He asks how it felt to eat Kent's children. The demon says that he's sorry and Father Thompson says that he has been saved. Dean and Sam plan to recreate the exorcism – using Abaddon.

Over lunch, Metatron and Castiel discuss the mess that Heaven is in and Castiel admits that it's his fault. Metatron says that he wrote down for God a set of instructions that can lock the angels into Heaven until order can be restored. He says that a warrior like Cas is the only one strong enough to complete the trials. Castiel agrees. Metatron informs Cas that the first trial involves cutting the heart out of their waitress, who is a Nephilim (the offspring of an angel and human). Castiel has to decide what is more important; the Nephilim (an “abomination,” according to Metatron), or their heavenly family. Sam and Dean dig up Abaddon, whom they had dismembered following Henry Winchester's death, and reassemble her except for her hands. She is still trapped with the devil’s trap bullet still in her head.

Crowley calls the guys and Abaddon is incensed to find out he is now the King of Hell. Sam and Dean leave the room to talk to Crowley, who tells them about Tommy Collins' death.

The guys figure there was nothing they could do about Tommy’s death and walk back into the warehouse, only to find Abaddon gone, along with her hands. The only thing left is the discarded bullet.

Crowley sends the boys an address in Prosperity, Indiana and the boys realize that it is yet another town where they saved someone, this time a girl named Jenny Klein who was targeted by two witches. They drive to the address, only to find Jenny, an avid baker, burned to a crisp in her oven. Crowley calls and announces that, unless they surrender, he is going to systematically kill everyone that they have ever saved, one every 12 hours. He gives them an address in Indianapolis and tells them they have one hour before the person there is killed.

Castiel and Metatron approach the waitress. She knows what they are and explains that she just wants to live her life. Castiel apologizes and approaches her with his blade, and the Nephilim fights both he and Metatron with little effort. Castiel manages to grab the blade and stabs her in the back completing the first “angel trial.”

The brothers drive to the Indianapolis address, and Sam is surprised to find Sarah Blake is the intended victim. Sam explains the situation to Sarah and they catch up while Dean thoroughly demon-proofs the hotel room. Crowley calls and counts down the last few seconds. Sarah falls on the floor, choking, and Crowley announces that his mother, a witch, taught him a few things. Dean and Sam look everywhere for the hex bag, but are too late and Sarah dies. Crowley tells them he plans to keep demons away from them, since he suspects they may need demons for the trials, and that, if they don't give up, he will continue killing everyone they’ve ever rescued. Dean slams the phone into the wall and the hex bag falls out of the handset onto the floor.

Back at the bunker, the brothers talk about Crowley’s ultimatum. Sam says that maybe they should take the deal and give up. Dean wants to fight and asks Sam if he is with him on it. Sam doesn’t respond.

Metatron: Look, I've been on sabbatical. I'm trying to play catch-up, but I have to talk to somebody about what's going on back home.

Castiel: And by home, you mean Heaven?
Metatron: Right. I've been looking around, crawling through a few divine nooks and crannies, and from what I can see, without the archangels, it's a mess up there. Open warfare.
Castiel: I thought Naomi was running things now.

Metatron: Is that what she told you? I mean, Naomi's a player -- don't get me wrong -- just one of many. There are factions upon factions, all fighting, betraying each other. It's just a matter of time before they start ripping each other apart. It's all broken.
Castiel: What did you mean, we can shut down Heaven?
Metatron: Oh, you know, the trials -- God's little 'pull in case of emergency.' The Leviathans get out of control, you put them in Purgatory. Demons get a little too demonic, toss 'em into Hell. Angels get uppity, slam the pearly gates.
Abaddon: Father Max Thompson, born October 12, 1910. Died August 5, 1958. Who do you think ripped him apart? Word got back to home office that Maxie was messing with things, so we made an example.It wasn't my most artful kill, but it was effective. And bonus -- before he died, he told me all about Josie Sands. I found her, and I rode her into the Men of Letters. And what I did to them, that was art.
Sarah: Yeah, I... his name is Ian. He works search and rescue. Guess I have a type. Our daughter, Bess -- she'll be one in a month.

Sam: That's, uh, great. I mean it. I'm really, uh... I'm really happy for you.
Sarah: Thanks, Sam. And what about you?
Sam: Me? Pretty much the same, I guess.

Sarah: No, you're not. You're not the same. Look, it's been years, and I can't even imagine the things you've been through. But I don't know. You just seem... more focused, confident, like... like you know what you want. You grew up, Sam. I do miss the old haircut, though.
Crowley: I thought of sending in a few of my bruisers, really letting them go to town. But then, well, trial one was kill a hellhound. trial two was rescue a soul from the pit. So, from here on, I'm gonna keep everything Hell-related -- demons, et cetera -- away from you. Safe side and all that -- plus, I just thought it seemed fitting. From what I understand, Sammy took that bird's breath away. What's the line? 'Saving people, hunting things -- the family business.' Well, I think the people you save, they're how you justify your pathetic little lives. The alcoholism, the collateral damage, the pain you've caused -- the one thing that allows you to sleep at night, the one thing is knowing that these folks are out there, still out there happy and healthy because of you, you great, big, bloody heroes!

A "clip show" refers to an episode of television which is based around clips from old episodes, usually by having characters reminisce, or dream about the past. They are cheap episodes to produce. The title here refers to the re-appearance of characters from past episodes: Tommy Collins from 1.02 Wendigo, Jenny Klein from 7.05 Shut Up, Dr. Phil, and Sarah Blake from 1.19 Provenance.
Dean finds that the Men of Letters files document every case of demonic possession and exorcism for the last 300 years including that of Ichabod Crane and Lizzie Borden.

Dean: Class 5 infernal event?
Sam: Yeah. See, the Letters had this whole ratings system. The Exorcist would have been a Class 2.

The Exorcist is a book and movie about a young girl possessed by a demon. Linda Blair, who famously portrayed the young girl, appeared in 2.07 The Usual Suspects as Detective Diana Ballard.
The case marked "weird!!!" by the Men of Letters was case 1138. 1138 has special significance to George Lucas fans. His first short film was Electronic Labyrinth: THX 1138 4EB, which was later later remade as the feature film THX 1138. Subsequently, the number 1138 occurs in many Lucas films including Star Wars.
Dean: Yeah, I think it's time we put Humpty Dumpty back together again.
Humpty Dumpty is a nursery rhyme about a anthropomorphic egg, that falls off a wall and cannot be put back together.
Dean: Dude, we got needles. We got thread. We've seen Young Frankenstein about a thousand times. Yeah, we're golden.
Young Frankenstein is a 1974 comedic take on Mary Shelly's Frankenstein, written and directed by Mel Brooks and starring Gene Wilder.
Crowley's phone number appears as 666 on Dean's caller ID. 666 is the Number of the Beast, as described in Revelation chapter 13.
Abaddon's disembodied hand moving around after escaping the box is reminiscent of Thing in The Addams Family.
Crowley: Now, you're probably wondering why my droogs aren't in there giving you the bums rush.
Droog is the Russian word for friend, and was most famously used as nadast slang in Anthony Burgess's novel A Clockwork Orange.
Crowley: tout-bloody-suite.
"Tout suite" means "right now" in French.

Minutiae

The exorcism Father Max uses to cure the demon is, "Exorcizamus te, omnis immundus spiritus hanc animam redintegra lustratus! lustratus!" combined with the application of purified blood. The exorcism translates as "We exorcise you, every impure spirit... Fix this soul... purified! purified!"
Donnelly Rhodes who plays the older Father Simon previously portrayed Shaw, the grown victim of a childhood wendigo attack in 1.02 Wendigo.
At the convenience store, Castiel buys things he knows Dean likes: beef jerky, beer, a copy of Busty Asian Beauties, and toilet paper. He tries to buy pie, but of course to no avail, continuing the running gag in which Dean never gets his pie.
The name of the reporter on the article about Tommy Collins' death is 'Christopher Cooper'. Christopher Cooper was property master for Supernatural.
Crowley uses the Supernatural books to research Sam and Dean's past cases and to find people they saved.
Crowley: What's the line? 'Saving people, hunting things. The family business.'
Dean used the line in 1.02 Wendigo when Sam asked why they were helping hunt the wendigo. Dean held up John's Journal and responded, "This is why. This book. This is dad’s single, most valuable possession. Everything he knows about every evil thing is in here, and he’s passed it on to us. I think he wants us to pick up where he left off, you know, saving people, hunting things. The family business."
Dean: We'll figure this out. We will. Man, we'll get it done. We'll kick it in the ass like we always do. Are you with me?
Kick it in the ass! was the catch-phrase of the late producer/director Kim Manners.
Deleted scene: The deleted scene from this episode included on the Season 8 DVD and Blu-ray is a short clip of Sam and Dean pulling up outside Jenny's house. Dean hands Sam the demon-killing knife and asks if he's ok. Sam responds that he's playing through the pain, and Dean praises him for that before loading a pistol with devil's trap bullets before climbing out of the Impala. Read the transcript here.
